Serious injury after vaccination: prepare a Canadian program application
Check eligibility, assemble medical records and follow Canada’s vaccine injury assistance program process.
Obtain needed care first. A financial-assistance application is separate from an adverse-event report to Canada Vigilance and does not by itself establish medical causation.
Confirm the right program
Verify that the vaccination was given in Canada and that the situation meets current criteria, including seriousness, permanence and timing. The federal government now administers applications; use its current program page.
Build a chronology
Record the vaccination date and place, product and lot when known, symptom onset, consultations, diagnoses, treatment, limitations and effects on work or daily activities. Do not make your own conclusion about causation.
Gather forms and authorizations
The program may request forms, consents, medical information and financial evidence. Keep a copy of everything, obtain confirmation of receipt and answer requests for more information by the stated deadline.
Keep other steps separate
Continue medical care and check relevant insurance, employment or disability benefits. A program decision is not medical advice, and merely reporting an event does not guarantee eligibility.
