Lost vaccination record in Quebec: how to retrieve proof and correct your file

How to request a copy of information from the Quebec Vaccination Registry, correct an error and deal with a vaccine that is missing from the record.

The Quebec Vaccination Registry does not always replace a paper vaccination booklet. It centralizes information sent to the Quebec health network, but a missing entry does not automatically prove that a vaccine was never received.

Request a copy

The Government of Quebec provides a process to request a copy of the vaccination information about you in the registry. Use the current form and identification requirements.

A vaccine is missing

Look for the paper record or proof from the clinic, pharmacy or provider.
Contact the place where the vaccine was given and ask whether the information was transmitted.
For a vaccine received outside Quebec, ask the health network how it can be validated and added when possible.

An entry is wrong

Do not edit proof yourself. Use the health-network correction process so the official registry is corrected at the source.

Travel, school or employment

Ask the requesting organization which proof it accepts. A registry copy, paper booklet and destination-specific certificate may not be interchangeable.
