Closed medical clinic: find your patient record and maintain care

Locate the record custodian, request access or transfer and protect continuity of treatment.

Urgent clinical need

Do not wait to recover the entire record before seeking urgent care, an essential prescription renewal or assessment of a concerning result. Use 811, a clinic or emergency care as appropriate.

Look for transfer notices

Check the telephone message, website, clinic door, emails and communications from the physician. When a practice closes, clinical records must be preserved or transferred under professional rules.

Identify the custodian

Look up the physician in the Collège des médecins directory and ask who holds the record. If several professionals worked at the clinic, their records may not have the same custodian. Public institutions and private clinics may use different processes.

Make a focused written request

Provide your identity, contact details, date range, priority documents and secure recipient. Ask for a copy and, if needed, transfer of the information required by the new professional. Keep proof of sending and the response.

Protect continuity of care

From memory, list medications, allergies, diagnoses, pending tests and specialists. Check Québec Health Booklet for information available there, without assuming it contains the entire clinical record.
