Error in a medical record: request a correction without erasing the history
Distinguish inaccurate facts from clinical opinion and make a precise, traceable correction request.
If the error could lead to the wrong medication, a missed allergy or unsafe care, tell the care team immediately. The formal written process can follow.
Compare the underlying documents
Obtain the note or result at issue and identify the exact date, author, field and disputed information. Attach reliable evidence when the error concerns identity, a date, prescription, test result or another verifiable fact.
Choose correction or comment
Factually inaccurate, incomplete or ambiguous information may support a rectification request. A professional opinion or clinical observation may not be erased; in that case, ask to have your disagreement or clarification added to the record.
Write to the responsible person
Send a signed request to the organization’s privacy officer or the professional holding the record. Describe the requested correction, reasons, evidence and prior recipients who should be notified if the correction is accepted.
Document the response
Keep the request, delivery proof, decision and corrected version. If the request is refused or unanswered, promptly confirm the remedy and deadline with the Commission d’accès à l’information, because the applicable regime depends on who holds the record.
