Signing is not admitting
The form is distributed free by the Groupement des assureurs automobiles. Responsibility is decided later by insurers using the Direct Compensation Agreement scale, so the report is used to document, not to judge.
What the form really asks
Driver identity and licence, vehicle make, model, year, plate and VIN, both insurers and policy numbers with the coverage period, visible damage with its exact location, the circumstances boxes, a sketch showing position and direction, named witnesses, and both signatures with one copy each.
The sketch
Three things make it readable: one arrow per vehicle for direction, a cross for the point of impact, and the street names with their direction. A rough sketch consistent with the ticked boxes is more useful than a precise one that contradicts them.
If the other driver refuses or leaves
You can complete the report alone: note the plate, the model, take photos, get witness details and declare the crash to your insurer. If the other driver leaves, it is a hit-and-run: note everything, call the police, and mention it in your declaration.

Notify your insurer or broker without delay and send the report; keep a copy and the photos for several years. You do not have to negotiate repayment directly with the other driver: a private deal can complicate your file.
