Pharmacy Medication Error in Quebec: Care, Correction and Complaint

Respond to a wrong product, dose, label or instruction without delaying care or losing evidence.

Severe symptoms or wrong dose taken

Call 911 for distress. Otherwise immediately contact the pharmacist or Poison Centre at 1-800-463-5060 as appropriate.

Do not improvise the next dose

Set the product aside without discarding or sharing it. Do not double or skip a dose to compensate without clinical direction. Keep the container, label and tablets.

Correct the treatment

Call the pharmacy with the name, dose, time taken, symptoms and regular medication. Request clear instructions, correction of the prescription and the name of the person handling the case.

Document consequences

Preserve photos, receipts, travel, lost income, care and communications. Request a record copy when needed and state facts without changing the packaging.

Choose the remedy

A discussion may correct the error. For concerning professional conduct, the Ordre des pharmaciens accepts a report or investigation request. Financial compensation requires a separate process.
