Pharmacy or regulated party: report loss or theft of controlled substances or precursors

Secure stock and people, reconcile inventory, notify police when required and send the regulatory report on time.

Priority

For theft in progress, a threat or exposure, call 911. Do not delay the initial report while waiting to know every missing quantity exactly.

Confirm the scope

Restrict access, preserve video and logs and determine whether anyone needs immediate medical or police response.

Prepare the information

Reconcile by product, DIN or name, strength, form, lot, quantity, discovery date and last reliable check.

Complete the process

Notify management, the designated person, police and Health Canada as required and use the current loss-or-theft form.

Keep follow-up records

Keep the report, police number, inventory adjustment, cause, preventive actions and any required update.
