Air, marine, rail or pipeline accident or incident: determine whether it must be reported to the TSB
Identify the mode, check the regulatory criteria and promptly submit the initial notice and any required report.
When danger is immediate, protect people and call emergency services. Reporting to the TSB does not replace 911 or duties owed to other authorities.
Identify the regime
Determine whether aviation, marine, rail or pipeline rules apply. Reportable events and responsible reporters differ by mode.
Preserve the facts
Record time, place, vehicles or facilities, damage, injuries, substances and witnesses. Do not disturb the site except for safety or when authorized.
Make the initial report
Contact the TSB as soon as possible by the quickest means when the criteria are met. Give verified facts and identify what remains unknown.
Complete follow-up
Submit the detailed form within the applicable period, keep confirmation and coordinate separate notices required by the operator or regulator.
