Possible vehicle, tire or car-seat defect: report it
Remove the product from danger, check recalls and report a reproducible safety problem to Transport Canada.
Stop using the vehicle, tire or seat if a failure could cause a crash or compromise restraint. Arrange towing when needed.
Describe the failure
Record symptoms, speed, weather, mileage, warning lights, injury or near miss. Do not clear codes or discard a part before preserving useful information.
Identify the product
Record VIN, make, model, year, manufacture date, part or seat number and retailer. Keep invoices, diagnosis and photos.
Check recalls
Search the recall database and contact the manufacturer or dealer. The absence of a known recall does not prevent a potential-defect report.

Use the form for vehicles, tires and child car seats. Describe a safety issue, not only a service dispute, and keep the assigned number.
